Patients with tumors measuring < 2 cm (greatest dimension) at specimen gross examination were excluded from the analysis because biopsies could not be collected from inside the demarcated tumor border. One case was excluded because of insufficient time to complete data collection prior to initiating formalin fixation of the specimen
