Cox models were adjusted for all variables in the table and year of diagnosis (continuous), marital status at diagnosis, tumor grade, neighborhood SES (quintile), and first course of treatment (chemotherapy (y/n), radiation (y/n), and surgery (y/n)). AJCC stage levels I through IV and unknown were included as a stratifying variable.
